IKWF 2010 Competitor Card Application
This is the required form to be turned to the Mat Masters when signing your child up for wrestling. This form will supply the required USAW card needed to compete and practice. FIRST YEAR WRESTLERS MUST ALSO SUPPLY A CERTIFIED COPY OF A BIRTH CERTIFICATE
